\nWe are happy to be able to serve the West Island community and beyond! We are pleased to say that we are accepting new patients, ages 0-110! Please complete the New Patient form once you have scheduled your appointment.<\/p>\n

Medical History
\nSo important! Please complete this form before the first time you visit us at the Clinic. We recommend setting aside 5-10 minutes to complete this form in the days preceding your appointment. This will eliminate the need for Donna and Christine to text or email you repeatedly before your appointment (unless you really want to hear from them, but still fill in the form, they can always communicate with you for other reasons too!)<\/p>\n
