Medical History
So important! Please complete this form before the first time you visit us at the Clinic. We recommend setting aside 5-10 minutes to complete this form in the days preceding your appointment. This will eliminate the need for Donna and Christine to text or email you repeatedly before your appointment (unless you really want to hear from them, but still fill in the form, they can always communicate with you for other reasons too!)

In order to facilitate a smooth running clinic and to be able to see all our patients in a timely manner, we ask that you read and sign the cancellation policy before coming to your first appointment.
