This Patient Info questionnaire asks for your contact information, private insurance information (if you have any) and medical history. We also ask that you sign the Cancellation Policy to help us have reasonable wait times for all our patients. These forms will help Dre Laflamme and her team to maintain their records and to provide the best possible care and reduce the risk of medical complications. It is in the patient’s best interest to carefully complete the entire form and to notify the dentist of any change in their health condition or contact information.

If you do not have access to any of these devices, no problem, please advise the receptionist before you arrive and they will have paper copies for you to fill out when you arrive. Preferably arrive 10 minutes early to give yourself time to do the paperwork.
